Role Overview

Palliative Care South East is seeking a Specialist Community Palliative Care Nurse to join its interdisciplinary clinical team. This position requires advanced clinical expertise and strong decision-making skills to support individuals living with complex health needs in community settings.

As a clinical expert within the team, the successful candidate will demonstrate strong problem-solving abilities, critical thinking, and the capacity to interpret and apply clinical data effectively. The role focuses on delivering specialist palliative care services directly to patients or providing consultative support to families, carers, and other healthcare professionals.

The nurse will also contribute to maintaining and improving clinical standards, ensuring that high-quality palliative care is consistently delivered across the service.

Key Responsibilities

The Specialist Community Palliative Care Nurse will play an important role in coordinating and managing complex care for patients and their families. Core responsibilities include:

  • Providing specialist palliative care within a community setting
  • Supporting patients and families through holistic care planning
  • Coordinating complex case management across multiple services
  • Collaborating with external healthcare providers and community organisations
  • Participating in rotational duties, including access, intake, and clinical escalation response
  • Supporting the delivery of safe, efficient, and responsive palliative care services

In addition to direct patient care, the nurse will act as a clinical leader and mentor, supporting less experienced staff members, including graduate nurses.

Leadership and Professional Contribution

This role extends beyond clinical care. The successful candidate will contribute to strengthening professional practice and service development within the organisation.

Responsibilities include:

  • Acting as a positive role model within the clinical team
  • Providing guidance and mentorship to junior nurses and colleagues
  • Supporting clinical education and skill development
  • Contributing to quality improvement initiatives and research activities
  • Serving as a key clinical resource within the organisation

By sharing expertise and supporting team development, the Specialist Community Palliative Care Nurse helps maintain high clinical standards across the service.

Required Skills and Experience

To succeed in this role, candidates should demonstrate strong clinical capability and a commitment to delivering compassionate community healthcare.

Essential requirements include:

  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Proven experience delivering coordinated care in collaboration with external healthcare providers
  • Strong interpersonal and communication skills
  • Computer literacy, including Microsoft Office and clinical electronic database systems
  • A valid National Criminal Record Check and Working with Children Check prior to employment
  • A current full Victorian Driver’s Licence

Experience in community nursing, palliative care, or complex case management will be highly regarded.
