About the Role
In this role, you will provide expert nursing care to patients with diabetes, helping to manage complex cases and prevent hospital admissions. Working within an integrated healthcare system, you will support patients in community settings, empowering them with the knowledge and tools needed to manage their condition effectively.
You will be encouraged to grow into an autonomous practitioner, delivering specialist diabetes care while collaborating closely with other healthcare professionals.
Key Responsibilities
- Deliver high-quality, patient-centred diabetes care in community settings
- Support patients with education, self-management, and treatment plans
- Work collaboratively with GPs, consultants, and multidisciplinary teams
- Monitor and manage complex diabetes cases
- Contribute to reducing hospital admissions through proactive care

Working Requirements
- Full UK driving licence and access to a vehicle is essential
- This is a countywide community-based role
- Generous mileage allowance provided, covering fuel and vehicle depreciation for NHS business use
What We’re Looking For
- Registered Nurse with valid NMC registration
- Experience in diabetes care or a strong interest in the specialty
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
