Key Responsibilities
Patient Care & Discharge Planning
- Conduct daily interdisciplinary rounds with real-time documentation and care board updates.
- Support safe and timely patient discharges by managing orders, identifying needs, and collaborating with clinical teams.
- Ensure patients and families understand discharge instructions, medications, and procedures.
Communication & Coordination
- Facilitate clear communication among interdisciplinary teams.
- Clarify care plans and escalate concerns to improve patient safety and quality of care.
- Provide education to staff on discharge processes and patient care best practices.
Safety & Quality
- Monitor patient needs, identify barriers to care, and implement safety measures.
- Support organizational improvement initiatives and uphold hospital quality standards.
- Perform staff nurse duties as needed while maintaining competency in clinical and technical skills.
Qualifications
Required Education & Experience
- Associate Degree in Nursing (ADN)
- 2+ years of bedside nursing experience
Preferred Education & Experience
- Bachelor of Science in Nursing (BSN)
Required Licensure & Certification
- Licensed Registered Nurse (LRN) with eligibility to work in Kansas (or compact license)
- Basic Cardiac Life Support (BLS) – American Heart Association
Preferred Certifications
- Medical/Surgical Nursing (RN,C)
- Gerontology (RN,BC)
- Progressive Care Nursing (Adult) – PCCN (AACN)
- Certified Medical-Surgical Registered Nurse (CMSRN) – AMSN
- Advanced Cardiac Life Support (ACLS) – AHA
