About the Role
As a Registered Nurse (RN) Case Manager, you will lead patient care with a personalized, hands-on approach. Working closely with an interdisciplinary team, you’ll assess patient needs, develop care plans, and provide ongoing support to ensure the best possible outcomes.
Work Schedule:
- Monday to Friday, 8:00 AM – 5:00 PM
- On-call rotation: Every 4th weekend
Key Responsibilities
- Conduct comprehensive patient assessments and create individualized care plans
- Collaborate with physicians and healthcare teams for coordinated care
- Promote patient independence through education and self-care training
- Supervise LPNs and Home Health Aides (HHAs)
- Maintain accurate, timely clinical documentation
- Monitor patient progress and communicate updates to care teams
- Participate in staff education and continuous professional development
- Provide on-call support to patients and families as needed

- Competitive Salary: $72,000 – $85,000 annually (transitioning to performance-based pay)

Qualifications
Required:
- Active Registered Nurse (RN) license in the state of practice
- Minimum 1 year of clinical nursing experience (or VP approval if less)
- Current CPR certification
- Valid driver’s license, reliable transportation, and insurance
Preferred:
- Experience in home health nursing, patient care management, or clinical case management
- Strong communication, organizational, and patient education skills
