About the Role
As a Community Hospital Nurse at Westbrook House, you’ll work in a nurse-led environment focused on holistic rehabilitation and therapy. You’ll support patients who need additional care before returning home, as well as those requiring end-of-life support.
This is a calm and supportive setting—but it also requires confidence, clinical judgement, and the ability to make independent decisions in complex situations.
Key Responsibilities
- Assess patients’ healthcare needs on admission
- Deliver and monitor high-quality, evidence-based nursing care
- Take professional accountability for patient care and outcomes
- Support rehabilitation, recovery, and end-of-life care
- Collaborate with multidisciplinary teams to ensure safe and effective care
- Provide compassionate, patient-centred support to individuals and families
Patient Group
You’ll care for a diverse range of patients, including:
- Older adults with complex health needs
- Individuals recently discharged from acute hospitals
- Patients with long-term conditions or physical disabilities
- Those requiring palliative or end-of-life care
About You
You are a dedicated and compassionate Registered Nurse who values meaningful patient interaction and high-quality care. You are confident working both independently and as part of a team, with strong clinical judgement and a commitment to continuous learning.
