You’ll play a key role in helping people recover after cardiac events, improve their long-term health, and regain confidence in managing their condition at home.
About the Role
As part of a friendly and experienced multidisciplinary team, you will contribute to the delivery of high-quality cardiac and pulmonary rehabilitation services. Your work will focus on supporting patients through recovery while promoting independence and long-term wellbeing.
Key Responsibilities
- Assess individual rehabilitation needs and develop safe, personalised care plans
- Deliver nurse-led cardiac rehabilitation interventions
- Support patients, families, and carers with clear, practical health education
- Collaborate with specialist nursing teams, community services, and secondary care providers
- Monitor patient progress, manage risks, and update care plans accordingly
- Maintain accurate clinical records and contribute to audits and service improvement
About You
We welcome both newly qualified and experienced nurses who are passionate about developing their skills in cardiac care and rehabilitation nursing.
You are a compassionate communicator who can support patients through complex health journeys while working effectively both independently and as part of a team.
Essential Criteria
- Registered Adult Nurse qualification
- Knowledge of cardiology services and evidence-based practice
- Strong communication skills, including handling sensitive information
- Ability to work collaboratively within multidisciplinary teams
- Basic IT skills and experience with clinical systems
- Ability to travel across the service area
