About the Role

The Primary Health Care Nurse (Rural/Remote) will provide advanced clinical nursing support and care coordination to general practitioners and other healthcare professionals. The role focuses on ensuring smooth day-to-day operations at the cohealth Bicheno practice while delivering high-quality patient care.

This position involves working closely with community members to provide preventative healthcare services, manage urgent care situations, and support patient treatment plans.

Graduate Registered Nurses interested in rural healthcare are also encouraged to apply, with additional support available for early-career professionals.

Key Responsibilities

As a Primary Health Care Nurse, your duties will include:

  • Conducting initial patient assessments and triage for urgent care presentations
  • Implementing preventative health checks and age-based health assessments
  • Developing and coordinating care plans and case conferences
  • Supporting general practitioners with treatment implementation and patient management
  • Providing professional and courteous assistance to patients, colleagues, and visitors
  • Ensuring care delivery aligns with the cohealth Code of Conduct and healthcare standards

Ideal Candidate

The successful candidate will be a motivated and compassionate Registered Nurse with strong clinical capabilities and a commitment to community healthcare.

Essential Requirements

  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Nurse Immuniser authorisation (or willingness to obtain within 6 months)
  • Advanced Life Support (ALS1) certification or willingness to obtain
  • Valid Tasmanian Driver’s Licence
  • Ability to work collaboratively in a multidisciplinary healthcare team

Experience in primary health or rural healthcare is beneficial but not mandatory.
