About the Role

As an MDS Nurse / MDS Coordinator, you will play a critical role in developing and maintaining comprehensive care plans for our residents. You will work closely with the nursing and clinical team to ensure timely, accurate, and individualized care that meets both resident needs and regulatory requirements.

Key Responsibilities:

• Develop and coordinate comprehensive care plans using the MDS process
• Monitor resident status and ensure timely involvement of nursing or clinical teams
• Maintain accurate MDS documentation and perform regular audits for compliance
• Assist with discharge planning and administrative reporting
• Promote resident comfort, dignity, and privacy in all interactions
• Stay current with federal and state regulations and professional standards
• Participate in departmental meetings, in-services, and continuous learning initiatives

Qualifications

Required:

• Current State License as a Nurse (RN or LPN)
• CPR Certification
• Proven MDS experience

Preferred:

• One year of nursing experience in long-term care
• Knowledge and training in all aspects of the MDS process
