- Working Pattern: 7-day rota (8:00 AM – 9:00 PM shifts)

About the Role
As a Community Staff Nurse, you’ll work as part of a dynamic district nursing team, delivering high-quality healthcare to patients in their homes and community settings.
You’ll support individuals with long-term conditions, complex care needs, and palliative care, ensuring they receive the right care in a familiar and comfortable environment.
Working alongside multidisciplinary teams, you’ll play a key role in assessing patient needs, planning care, and delivering tailored interventions that improve health outcomes and quality of life.
Key Responsibilities
Patient Care & Clinical Practice
- Deliver person-centred nursing care in patients’ homes and clinics
- Assess, plan, and evaluate care for a diverse patient population
- Support patients with long-term conditions and complex health needs
- Provide palliative and end-of-life care with compassion and dignity
Team Collaboration
- Work closely with primary healthcare teams and community partners
- Contribute to integrated care planning and service delivery
- Use agile technology to support efficient and responsive care
Flexible Service Delivery
- Participate in a 7-day rota, ensuring continuous care coverage
- Adapt to changing patient needs within a fast-paced community setting
Career Development Opportunities

Essential Requirements
- Registered Nurse (NMC registered)
- Strong clinical and communication skills
- Ability to work independently and as part of a team
- Willingness to travel across the Bradford district
