About the Role

As part of an integrated community healthcare service, you will provide high-quality nursing care to adults with a wide range of health needs. Your work will support patients in their own homes and residential settings, helping them maintain independence and wellbeing.

You’ll be joining a supportive and collaborative team that works across Integrated Nursing and Conditions services, giving you the opportunity to grow your skills while contributing to continuous service improvement.

Key Responsibilities

Person-Centred Patient Care

  • Conduct holistic assessments to understand each patient’s needs
  • Develop and implement tailored care plans
  • Deliver safe, effective, and compassionate nursing care

Collaborative Working

  • Work closely with multidisciplinary teams and external partners
  • Support coordinated and seamless community care delivery
  • Build strong relationships with patients, families, and carers

Clinical Documentation & Systems

  • Accurately update and manage the Electronic Patient Record (EPR)
  • Maintain clear and up-to-date patient records

Patient & Family Support

  • Educate and guide patients and carers in managing health conditions
  • Promote independence, wellbeing, and long-term health outcomes

Skills & Experience Required

  • Registered Nurse with a valid NMC registration
  • Strong clinical, communication, and assessment skills
  • Ability to work effectively within a team environment
  • Interest in community nursing (open to both newly qualified and experienced nurses)
  • Willingness to learn, adapt, and grow professionally
