About the Role
As a Children’s Community Nurse (CCN), you will provide specialist nursing care across a variety of home and community settings. You will be responsible for assessing patient needs, planning and delivering care, and ensuring ongoing evaluation to achieve the best outcomes.
Working both independently and as part of a multidisciplinary team, you will support children with complex health needs while promoting family-centred care.
Key Responsibilities
- Deliver high-quality, evidence-based nursing care to children in their home environments
- Conduct comprehensive assessments and develop personalised care plans
- Implement and evaluate nursing interventions, ensuring accurate documentation
- Work as an autonomous practitioner while supporting team coordination
- Collaborate with hospitals, GPs, and other healthcare professionals
- Promote family-centred care and empower families in managing care needs
- Maintain strong links between community and hospital-based services
- Support and mentor junior staff and student nurses during placements
- Contribute to continuous service improvement and clinical excellence
Skills & Experience Required
- Registered Children’s Nurse with valid NMC registration
- Experience in paediatric nursing (community or acute setting)
- Strong clinical assessment and decision-making skills
- Ability to work independently and manage caseloads effectively
- Excellent communication and interpersonal skills
- A compassionate, family-focused approach to care
- Willingness to learn and develop within a community setting
