As a Band 5 Mental Health Nurse, you will play a key role in supporting individuals with a range of mental health needs within the community. Working as part of a multidisciplinary team, you’ll contribute to delivering high-quality, coordinated care that promotes recovery and independence.

This role combines clinical practice, patient engagement, and continuous learning, helping you build confidence in community mental health settings.

Key Responsibilities

Deliver Person-Centred Care

  • Develop and implement recovery-focused care plans
  • Work closely with service users, families, and carers
  • Provide psychoeducation on mental health conditions and treatments

Risk Assessment & Care Coordination

  • Assess and manage clinical risks effectively
  • Monitor progress and evaluate care interventions
  • Escalate concerns and work collaboratively with senior clinicians

Clinical & Community Support

  • Support medication administration and monitoring
  • Carry out triage and assist with referral assessments
  • Signpost individuals to appropriate external services

Team Collaboration

  • Work closely with multidisciplinary teams and partner services
  • Participate in duty rota and respond to urgent queries
  • Build strong working relationships across services

Professional Development

  • Maintain accurate clinical records and documentation
  • Engage in supervision, reflective practice, and ongoing training
  • Contribute to service improvement, audits, and quality initiatives

Requirements

Essential:

  • Registered Mental Health Nurse (RMN) with valid NMC registration
  • Evidence of continuous professional development (CPD)
  • Experience in mental health or substance misuse settings
  • Strong communication and clinical assessment skills
  • Ability to assess, plan, and deliver effective care

Desirable:

  • Training in psychosocial interventions (CBT, DBT, Motivational Interviewing)
  • Experience working with complex or acute mental health cases
  • Physical health skills such as ECG or venepuncture
  • Experience working with diverse or underserved populations

Working Pattern

  • Full-time role with participation in a duty rota
