Role Details

The Aboriginal and Torres Strait Islander Health Unit at Sarina Hospital is currently seeking applications for a Clinical Nurse – Potentially Preventable Hospitalisations (PPH) Team to join the team on a permanent basis.

This role offers an opportunity to support culturally responsive healthcare while helping improve outcomes for First Nations communities across the region.

Your Opportunity

As a Clinical Nurse within the Potentially Preventable Hospitalisations (PPH) Team, you will work in an advanced nursing role focused on improving health outcomes for First Nations people living with or at risk of chronic conditions.

This role requires a registered nurse with specialised knowledge in chronic disease management and community-based care.

Key responsibilities include:

  • Delivering advanced clinical nursing care for individuals managing chronic health conditions
  • Assessing, planning, and coordinating treatment to support improved health and functional outcomes
  • Promoting best-practice nursing care tailored to the needs of First Nations communities
  • Supporting prevention, early intervention, and health promotion initiatives
  • Leading strategies to reduce Potentially Preventable Hospitalisations in line with national Closing the Gap initiatives
  • Ensuring clinical services are culturally appropriate and community-focused
  • Providing outreach support for First Nations patients living in the Mackay and Sarina areas who are managing or at risk of chronic disease
  • Strengthening access to primary healthcare services and improving long-term health outcomes

About the Team

The Aboriginal and Torres Strait Islander Health Unit plays a critical role within Mackay Hospital and Health Service.

The team focuses on developing and delivering culturally safe healthcare services that support the needs of Aboriginal and Torres Strait Islander people and their families.

By working closely with healthcare providers, community organisations, and patients, the unit helps ensure healthcare services are respectful, inclusive, and responsive to community needs.
