About the Role
In this role, you will provide clinical support through telehealth, helping customers stay safe and independent at home. You will assess clinical concerns, guide decision-making, and collaborate with a wider care network to ensure high-quality support.
Key Responsibilities
- Monitor and triage clinical communications and wellbeing updates
- Complete clinical assessments and care reviews
- Support customers and families with informed care decisions
- Assist with palliative care planning and advance care discussions
- Respond to clinical incidents and escalations
- Contribute to quality improvement and clinical governance initiatives
- Provide clinical insights across the wider care team and provider network
- Ensure practice aligns with the Aged Care Act, Quality Standards, and clinical governance frameworks
Skills and Experience
- Current AHPRA registration as a Registered Nurse
- Valid First Aid certification
- Experience in home and community care, preferably within aged care
- Strong clinical judgement and case management skills
- Excellent communication and customer service abilities
- A compassionate, people-focused approach to care
Experience with self-managed or consumer-directed care models is an advantage.
