Key Responsibilities of an Admission Nurse
Patient Admission & Education
- Meet with patients and families to explain hospice services and care plans.
- Conduct comprehensive admission consultations with empathy and clarity.
Clinical Assessment & Care Planning
- Perform thorough patient assessments during admission.
- Coordinate initial care plans, including nursing visits, medications (RX), and durable medical equipment (DME).
- Identify needs for additional services such as social work, chaplain support, bereavement care, and volunteers.
Collaboration & Documentation
- Present patient updates to the Interdisciplinary Team (IDT).
- Maintain accurate and compliant documentation according to organizational policies.
Leadership & Support
- Train and support other admission staff when needed.
- Conduct supervisory visits for LVN and CNA team members.
- Occasionally perform nursing or “tuck-in” visits to ensure patient comfort.
Qualifications
Required Credentials
- Active Registered Nurse (RN) license in Texas
- Valid driver’s license
Experience
- Minimum 1 year of nursing experience
- Case management experience required
- Preferred: 2+ years of hospice case management experience
Skills
- Strong communication and interpersonal skills
- Proficiency in computer systems and Microsoft Office
- Ability to provide compassionate, patient-centered care
