Role Overview: Hospice Nurse

Key Responsibilities of a Hospice Nurse

As part of the hospice care team, the Hospice Nurse provides holistic nursing care while collaborating with physicians, caregivers, and support staff to deliver personalized treatment plans.

Patient Care and Clinical Responsibilities

• Provide high-quality end-of-life nursing care to patients with life-limiting conditions
• Assess patient and family care needs and implement individualized care plans
• Collaborate with physicians and interdisciplinary care teams to ensure coordinated treatment
• Monitor patient conditions and adjust care strategies as needed
• Educate patients and families on infection control, symptom management, and home care techniques
• Maintain accurate and timely documentation using Electronic Medical Records (EMR)

Family Support and Guidance

• Provide emotional and educational support to families throughout the care journey
• Help families maintain patient comfort and independence whenever possible
• Offer compassionate support during the patient’s final stages of life

Quality and Team Collaboration

• Participate in quality improvement programs to enhance patient outcomes
• Supervise and guide Patient Care Partners, volunteers, and support staff

Education and Qualifications

Required Education

• ADN or BSN from an accredited nursing institution

Professional Experience

• Minimum two years of nursing experience or equivalent combination of education and clinical practice

Required Licensure

• Active Maryland Registered Nurse (RN) license
