Job Description
We are looking for an experienced Registered Nurse to join our team as a Care Partner. This role offers a different approach to nursing—focusing on care coordination, resident support, and family engagement rather than traditional shift-based duties.
As a Care Partner, you will play a key role in helping residents and their families navigate care options, funding arrangements, and healthcare services. Your work will contribute to high-quality, person-centred care while ensuring residents receive the support they need to live safely and comfortably.
About the Role
In this position, you will coordinate and oversee care services while building strong relationships with residents, families, and healthcare providers.
Key Responsibilities
- Conduct resident assessments and develop personalised care plans and budgets
- Coordinate safe, evidence-based care that aligns with regulatory and quality standards
- Build strong relationships with residents and families through regular wellbeing check-ins
- Provide guidance on care options and funding arrangements
- Connect residents with GPs, allied health professionals, hospitals, pharmacies, and government services
- Participate in case conferences and multidisciplinary care meetings
- Maintain accurate documentation and ensure compliance with Aged Care Quality Standards
- Support quality improvement initiatives, risk management, and incident reporting
- Contribute to a positive, collaborative team environment
About You

Essential Requirements
- Registered Nurse qualification with a minimum of five years’ clinical or community care experience
- Current AHPRA registration
- Strong care coordination, problem-solving, and communication skills
- Ability to work independently while collaborating with a broader healthcare team
- Commitment to quality care, safety, and continuous improvement
