As a Practice Nurse, you’ll be at the heart of delivering high-quality, patient-centred care across two vibrant sites. This is a varied and rewarding role where you’ll support patients with long-term conditions, lead nurse-led clinics, and play a key role in preventative healthcare.

You’ll work within a multidisciplinary team of GPs, Pharmacists, HCAs, Paramedics, and Nursing Associates—ensuring truly joined-up care for every patient.

Key Responsibilities

Clinical Care & Patient Management

  • Manage long-term conditions such as diabetes, asthma, and COPD
  • Conduct routine reviews and deliver proactive, preventative care
  • Run chronic disease clinics and refer patients to specialist services when required

Women’s Health & Immunisations

  • Perform cervical cytology (smear tests) and manage results
  • Administer childhood immunisations, travel vaccines, and flu vaccinations
  • Provide contraception advice and sexual health support

Health Promotion & Preventative Care

  • Promote patient education and self-management strategies
  • Support Quality and Outcomes Framework (QOF) targets and enhanced services
  • Deliver evidence-based care aligned with NICE and NSF guidelines

Clinical Support & Collaboration

  • Request and interpret pathology results
  • Maintain accurate clinical records and infection control standards
  • Work closely with the wider clinical team to deliver integrated care

Leadership & Development

  • Support and mentor HCAs and phlebotomists
  • Contribute to service improvement and innovation within the practice

What We’re Looking For

  • Registered Nurse (RGN) with current NMC registration
  • Experience in GP or primary care settings
  • Confident in managing chronic conditions and leading nurse-led clinics
  • Strong communication and teamwork skills
  • Passion for patient-centred care and reducing health inequalities
  • Proactive, adaptable, and committed to continuous learning
