Key Responsibilities

Wound Care Coordination

• Serve as the primary coordinator of the center’s wound care team.
• Oversee the application and management of our comprehensive skin management program.
• Lead weekly interdisciplinary wound team rounds, including measurements, care plan reviews, and treatment recommendations.

Patient Assessment & Documentation

• Identify and maintain accurate records of pressure ulcers and other skin alterations.
• Complete Braden risk scales upon admission and implement preventive interventions.
• Document all treatments, physician orders, and care plans accurately in electronic health records.

Data Analysis & Risk Management

• Collect, analyze, and share pressure ulcer data with nursing staff and healthcare practitioners.
• Identify center practices and risk-minimizing measures to improve patient safety.
• Support continuous quality improvement initiatives related to wound care and skin management.

Qualifications

• Current, active license as RN, LVN, or LPN in the state of Texas.
• Proven experience in wound care, skin management, or related nursing roles.
• Strong interpersonal skills to collaborate with interdisciplinary teams and educate patients and families.
• Attention to detail, critical thinking, and commitment to patient-centered care.
