Key Responsibilities
Wound Care Coordination
- Serve as the primary coordinator of the center’s wound care team.
- Oversee the application and management of our comprehensive skin management program.
- Lead weekly interdisciplinary wound team rounds, including measurements, care plan reviews, and treatment recommendations.
Patient Assessment & Documentation
- Identify and maintain accurate records of pressure ulcers and other skin alterations.
- Complete Braden risk scales upon admission and implement preventive interventions.
- Document all treatments, physician orders, and care plans accurately in electronic health records.
Data Analysis & Risk Management
- Collect, analyze, and share pressure ulcer data with nursing staff and healthcare practitioners.
- Identify center practices and risk-minimizing measures to improve patient safety.
- Support continuous quality improvement initiatives related to wound care and skin management.
Qualifications
- Current, active license as RN, LVN, or LPN in the state of Texas.
- Proven experience in wound care, skin management, or related nursing roles.
- Strong interpersonal skills to collaborate with interdisciplinary teams and educate patients and families.
- Attention to detail, critical thinking, and commitment to patient-centered care.
