About the Role
Make an Impact Through Clinical Accuracy & Care Planning
We are seeking an experienced MDS Nurse (Registered Nurse preferred) to lead the assessment, coordination, and documentation of resident care. This role is essential in ensuring compliance with state and federal regulations while supporting effective patient care management.
Key Responsibilities
- Conduct comprehensive MDS assessments (admission, quarterly, significant change, discharge)
- Collect and analyze clinical data related to residents’ physical, psychological, and social health
- Develop and update individualized care plans based on assessment findings
- Collaborate with the interdisciplinary team (IDT) including physicians, therapists, and social workers
- Ensure accurate documentation for proper Medicare and Medicaid reimbursement
- Monitor care plan effectiveness and implement necessary improvements
- Support quality assurance and regulatory compliance initiatives
- Train and guide staff on MDS processes and documentation standards
- Communicate effectively with residents and families to align care goals
What We’re Looking For
Skills & Qualifications
We’re looking for a highly organized and analytical MDS Nurse who can balance clinical expertise with strong communication skills.
Required Qualifications
- Graduate of an accredited nursing program
- Current Oklahoma RN license
- Valid CPR and First Aid certification
- Proven experience with MDS assessment and documentation
- Knowledge of long-term care regulations, Medicare, and Medicaid
Preferred Skills
- Previous experience in nursing home or long-term care settings
- Strong critical thinking and problem-solving abilities
- Ability to lead care plan meetings and coordinate across teams
- Excellent organizational and record-keeping skills
