About the Role: Registered Nurse – Home Care Case Manager
The Registered Nurse – Home Care Case Manager plays a critical role in coordinating healthcare services for patients living in the community. Working closely with healthcare professionals, caregivers, and families, you will develop and manage care plans that support patients with complex health needs.
This position requires strong clinical judgment, leadership, and communication skills to ensure that patients receive safe, efficient, and coordinated care.
Key Responsibilities
Patient Assessment and Care Planning
- Conduct comprehensive health assessments for community-based patients
- Develop individualized care plans tailored to each patient’s medical and social needs
- Continuously evaluate and update care plans based on patient progress
Community Care Coordination
- Collaborate with interdisciplinary healthcare teams, including physicians, therapists, and social workers
- Coordinate access to community resources and healthcare services
- Facilitate smooth hospital discharges and prevent avoidable hospitalizations
Patient Advocacy and Education
- Educate patients and families on health management, treatment plans, and preventive care
- Provide guidance on medication management and home care strategies
- Advocate for patient safety and quality healthcare services
Leadership and Clinical Excellence
- Support integrated healthcare delivery within the community sector

Work Location and Schedule
This Registered Nurse role is based at Red Deer Bremner Avenue Community Health Centre in Red Deer, Alberta.
Position Details
- Employment Type: Regular Full-Time
- Start Date: March 26, 2026
- Hours per Shift: 7.75 hours
- Shift Pattern: Days, evenings, and weekends
- Work Cycle: 12-week rotation
A valid driver’s license and access to a vehicle are required to support community visits and patient care coordination.
Salary and Compensation
Healthcare professionals in this role receive competitive pay aligned with industry standards.
- Minimum Salary: $43.26 per hour
- Maximum Salary: $59.21 per hour
Provisional permit holders will receive $39.58 – $54.16 per hour until full licensing requirements are completed.
Required Qualifications
To succeed as a Registered Nurse in this community-focused role, candidates should meet the following requirements:
Education and Certification
- Completion of an accredited nursing education program
- Eligible for registration with the College of Registered Nurses of Alberta (CRNA)
- Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) certification
If a candidate has not practiced as a Registered Nurse within the past five years, completion of refresher courses such as CRNA Medication Management and NextGenU Health Assessment Certification may be required.
Experience and Skills
Required Experience
- Minimum 3 years of community or home care nursing experience
- Strong understanding of home care programs and acute care environments
- Excellent communication skills in complex healthcare settings
- Ability to work independently and collaboratively within healthcare teams
Preferred Qualifications
- Experience with Connect Care, RAI-HC, and electronic medical documentation systems
- Knowledge of case management practices and community healthcare resources
- Experience in wound care, patient education, and home therapy programs
- Bachelor’s degree in Nursing (BScN) preferred
