Location and Role Details

Location: Aveo Northern Beaches Communities – Avalon Beach, East Lindfield, Mosman and St Ives
Employment Type: Permanent Full-Time
Reports To: Regional Home Care Manager

The Opportunity

Aveo Care at Home delivers personalised support through the government-funded Support at Home program, helping older Australians maintain independence while living in their own homes.

As a Care Partner (Clinical), you will manage a portfolio of residents and play a key role in coordinating home care services. Your role involves visiting residents in their homes, assessing their needs, and creating tailored care plans that support their health, wellbeing, and independence.

You will also guide residents and families through the My Aged Care system, ensuring they can access the services and funding available to them.

Your Key Responsibilities

As a Care Partner (Clinical), you will work closely with residents, families, and healthcare providers to ensure each person receives the support they need.

Your responsibilities will include:

  • Meeting with prospective residents and families to understand their care needs
  • Providing clear guidance on home care services and available funding options
  • Developing personalised home care plans for each resident
  • Coordinating support services and monitoring care delivery
  • Maintaining accurate documentation, reports, and care records
  • Providing exceptional customer service to residents, families, and health professionals
  • Collaborating with external healthcare providers within the community
  • Promoting Aveo Care at Home services through community engagement and events
  • Assisting residents with transitions and support throughout their My Aged Care journey

What You’ll Bring

We’re looking for a compassionate and organised healthcare professional who is passionate about supporting older Australians in their homes.

Essential Requirements

  • Current Registered Nurse AHPRA registration
  • Valid Driver’s Licence
  • Two COVID-19 vaccinations
  • Strong communication and customer service skills
  • Ability to work independently while managing a caseload

Preferred Experience

  • Experience in home care or community care services
  • Previous roles such as Community Registered Nurse, Case Manager, or Care Coordinator
  • Knowledge of My Aged Care, ACAT assessments, or Home Care Packages
  • Understanding of the Support at Home program
