Are you a Registered Nurse or Enrolled Nurse looking to make a real difference in aged care? Access Aged Care is seeking a dedicated nurse to support a newly onboarded aged care home in Cooranbong, NSW.

In this unique role, you’ll work closely with visiting GPs and healthcare teams, helping to identify early changes in residents’ health and ensuring timely clinical care. By strengthening communication between residents, families, facility staff, and doctors, you’ll play a vital role in improving health outcomes and raising the standard of care in aged care facilities.

About the Role

As part of our clinical team, you will provide on-site nursing support during GP visits, assist with specialist consultations, and advocate for residents’ healthcare needs.

This role focuses on clinical coordination and resident advocacy, rather than manual handling, allowing nurses to concentrate on delivering high-quality clinical care.

Key Responsibilities

  • Provide on-site clinical support during GP care reviews and follow-up visits
  • Facilitate telehealth consultations with geriatricians and other medical specialists
  • Monitor and identify changes in residents’ health conditions and escalate concerns when required
  • Build strong relationships with residents and their families, keeping them informed about care plans
  • Maintain accurate clinical documentation, follow-ups, and audit support
  • Assist with incident reviews and quality improvement processes
  • Work closely with aged care facility staff, supported by a Nursing Regional Manager and administrative team

You’ll also gain valuable exposure to different aged care environments, helping broaden your clinical experience.

Essential Requirements

  • Current AHPRA registration as a Registered Nurse or Enrolled Nurse
  • Minimum two years of postgraduate nursing experience
  • Strong clinical judgement and critical thinking skills
  • Excellent communication when working with residents, families, and healthcare teams
  • Ability to work autonomously while maintaining collaborative relationships
  • Strong organisational skills and attention to detail
  • Confidence using clinical systems and digital healthcare tools
  • Reliable vehicle and ability to travel locally across the Central Coast
