Position Overview
The Staff Nurse in Transitional Care is responsible for delivering comprehensive nursing services to patients and families. Key Responsibilities of a Staff Nurse
A Staff Nurse plays a central role in coordinating patient care and supporting healthcare teams. Key responsibilities include:
Deliver Comprehensive Patient Care
Utilize the professional nursing process—assessment, planning, implementation, and evaluation—to deliver effective and individualized patient care.
Medication Administration and Clinical Procedures
Safely administer medications and perform nursing procedures requiring advanced clinical knowledge, judgment, and technical skill.
Leadership and Team Supervision
Provide direction and support to licensed practical nurses and nursing assistive personnel, ensuring that delegated tasks meet clinical and regulatory standards.
Transitional Care Coordination
Act as the primary nurse and total patient caregiver, coordinating treatment plans and supporting patients as they transition between different levels of care.
Clinical Training and Mentorship

Education Requirements

Licensure and Certifications
