Position Overview
The Health Care Management Nurse I / II position plays a critical role in evaluating medical eligibility for state-supported long-term care programs. Level I:
$58,055 per year + Full State Benefits Package
Level II:

Key Responsibilities of a Health Care Management Nurse
A Health Care Management Nurse is responsible for conducting detailed medical assessments and supporting individuals seeking long-term care services.
Comprehensive Medical Assessments
Conduct telephone, electronic, or in-person home visits to evaluate applicants and gather detailed health information required for program eligibility.
Long-Term Care Eligibility Evaluation
Review and assess medical eligibility for programs such as:
- Title 19 Nursing Home Admissions
- State Plan Personal Care Services
- ADvantage Waiver Program
These evaluations help determine the level of care required and ensure applicants receive appropriate healthcare support.
Patient Needs Identification
Analyze assessment findings to identify medical and support needs while making recommendations for suitable healthcare services and long-term care options.
Documentation and Reporting
Maintain accurate documentation of patient assessments and ensure compliance with established healthcare standards and regulatory requirements.
Collaboration with Social Services
Assist with Adult Protective Services evaluations when requested and collaborate with multidisciplinary teams to address complex healthcare situations.
Minimum Qualifications
Health Care Management Nurse – Level I
- Valid permanent Oklahoma Registered Nurse (RN) license approved by the Oklahoma Board of Nursing or through the Registered Nurse Licensure Compact (eNLC)
- Minimum two years of professional nursing experience
Health Care Management Nurse – Level II
- Valid permanent Oklahoma Registered Nurse (RN) license approved by the Oklahoma Board of Nursing or through the Registered Nurse Licensure Compact (eNLC)
- Minimum three years of professional nursing experience

This role requires extensive travel within the assigned region to complete home visits and assessments.
Applicants must:
- Possess a valid driver’s license
- Maintain required car insurance
- Be able to respond to short-notice home visits
- Lift up to 15 lbs occasionally (for equipment transport)
- Maintain reliable home internet for telework when required
