The Wellness Nurse is responsible for overseeing residents’ health and well-being while ensuring the highest standards of clinical care and service delivery. This role combines hands-on nursing, care coordination, and leadership support to promote a safe, supportive, and engaging living environment.
You will collaborate closely with healthcare teams, families, and physicians to ensure each resident receives personalized and responsive care.
Key Responsibilities
Resident Care & Health Coordination
- Assess and monitor residents’ ongoing healthcare needs
- Conduct regular wellness visits and track changes in health status
- Maintain accurate and up-to-date clinical documentation
- Respond to incidents such as falls or medical concerns with proper follow-up
- Communicate effectively with families and healthcare providers
Medication Management
- Oversee medication administration and ensure compliance with prescribed treatments
- Maintain accurate Medication Administration Records (MAR/TAR)
- Conduct monthly audits and ensure safe medication practices
- Support and guide team members in medication procedures
Clinical Quality & Compliance
- Ensure adherence to healthcare regulations and safety standards
- Participate in quality assurance and clinical review processes
- Support infection control protocols and resident safety measures
- Assist in implementing care plans related to nutrition, mobility, and overall wellness
Team Support & Collaboration
- Provide clinical guidance and support to care staff
- Assist in staff training and performance monitoring
- Participate in team meetings and continuous improvement initiatives

Qualifications & Requirements
- Diploma or degree in Nursing from an accredited institution
- Valid LPN/LVN license (as per state/provincial regulations)
- Minimum 1 year of experience in assisted living, hospital, or long-term care
- Knowledge of healthcare regulations and clinical best practices
- Proficiency in basic computer applications (MS Office and healthcare systems)
- Flexibility to work evenings, weekends, and varied shifts
Work Schedule
- Sunday to Thursday
- 1:45 PM – 10:15 PM
