At Locala Health & Wellbeing, our Hospital at Home Service is transforming how acute care is delivered. Instead of hospital stays, we provide advanced clinical care at home, helping patients recover in familiar surroundings while reducing unnecessary admissions.
As a Community Frailty Nurse, you’ll be at the forefront of this shift—delivering compassionate, high-quality care where it matters most.
You’ll join a forward-thinking multidisciplinary team supporting patients with acute and complex needs across the Kirklees community. Working closely with Acute Consultants, GPs, and healthcare professionals, you will help prevent hospital admissions and enable safe, early discharge.
This service operates as part of the Calderdale, Kirklees and Wakefield (CKW) integrated care system, ensuring coordinated, patient-centered care across the region.
Key Responsibilities
Deliver Acute-Level Care at Home
- Provide advanced nursing care to patients with frailty and complex conditions
- Assess, monitor, and respond to changing clinical needs
- Support hospital avoidance and early discharge pathways
Collaborate Across Teams
- Work alongside senior nurses, physiotherapists, and healthcare assistants
- Partner with Acute Trust clinicians and GPs
- Contribute to holistic, patient-focused care plans
Support Digital & Remote Care
- Engage with remote monitoring systems and digital tools
- Maintain accurate and timely clinical documentation
- Adapt to innovative community healthcare models
What We’re Looking For
- Registered Nurse (NMC) with experience in acute, frailty, or community care
- Strong clinical assessment and decision-making skills
- Ability to work independently in a mobile/community setting
- Excellent communication and teamwork abilities
- A proactive, patient-focused mindset
Essential Requirements
- Full UK driving licence with access to a vehicle and business insurance
- Ability to travel independently across Kirklees
- Flexibility to work within a 7-day service schedule
