About the Role: Registered Nurse Community Nursing

The Registered Nurse Community Nursing role focuses on delivering patient-centered healthcare services within community settings. Working closely with multidisciplinary teams, community nurses coordinate, delegate, and lead clinical care for patients with complex health needs.

This position involves providing post-acute care, primary health services, and support through the Community Home Support Program (CHSP) to individuals and families across the Albury community.

Key Responsibilities

As a Community Registered Nurse, your responsibilities will include:

• Conducting comprehensive patient assessments and care planning
• Delivering complex clinical care including wound management and catheter care
• Coordinating care with multidisciplinary healthcare teams
• Administering IV antibiotics and CVAD care when required
• Maintaining accurate clinical records and documentation
• Supporting patients with acute, chronic, and complex healthcare needs
• Providing referrals to appropriate healthcare services when necessary

This role requires strong clinical expertise, leadership skills, and the ability to deliver safe, effective care in a community-based environment.

Required Qualifications and Experience

To succeed in this Registered Nurse Community Nursing position, candidates should meet the following essential criteria:

Professional Requirements

• Current Nursing/Midwifery registration with AHPRA (Australian Health Practitioner Regulation Agency)
• Minimum 3 years of postgraduate nursing experience in an acute care setting
• Strong knowledge of primary health care principles
• Experience with wound care, IV therapy, catheter management, and care coordination
