As a Registered Nurse within our Integrated Care Team, you will play a key role in delivering patient-centred care across both planned and urgent pathways. You’ll work closely with multidisciplinary teams to assess, implement, and evaluate care tailored to individual needs.
This role focuses on supporting housebound patients, ensuring they receive safe, responsive, and compassionate care in a familiar environment.
Key Responsibilities

Requirements
Essential Qualifications
- Valid NMC (Nursing and Midwifery Council) registration
- Evidence of ongoing professional development
- Previous experience in a healthcare setting
- Full UK driving license with access to a car
Desirable Skills
- Community nursing experience
- Palliative care experience
- Leg ulcer management training
Salary & Benefits
- £32,073 – £39,043 per year

Working Hours
- Full-time: 37.5 hours per week
- Flexible shifts between 8:00 AM – 8:00 PM
- Rotational schedule across 7 days
