About the Role

The Alpine Health District Nursing Service (DNS) delivers contemporary, evidence-based nursing care aligned with the organisation’s philosophy, policies, and commitment to community wellbeing.

Our district nurses support individuals through several government-funded healthcare programs, including:

  • Commonwealth Home Support Program (CHSP)
  • Victorian Home and Community Care Program for Younger People (HACC-PYP)
  • Department of Veterans’ Affairs (DVA) services

District nurses may also provide services through additional care programs such as:

  • Hospital in the Home (HITH)
  • Transitional Care Program (TCP)
  • Home Care Packages
  • Short-Term Support programs

In this role, you will deliver holistic nursing care within patients’ homes, focusing on clinical assessment, health promotion, care coordination, and supporting patients to maintain independence wherever possible.

Key Responsibilities

As a District Nurse at Alpine Health, your responsibilities will include:

  • Providing comprehensive clinical assessments and personalised care plans
  • Delivering professional nursing care in home and community settings
  • Supporting patients to maintain independence and improve overall wellbeing
  • Coordinating care with multidisciplinary healthcare teams
  • Monitoring patient progress and adjusting care strategies when required
  • Educating patients and families on health management and recovery

Skills and Experience

To succeed in this position, you will bring strong clinical expertise combined with a passion for community healthcare.

Essential Requirements

  • Registered Nurse with minimum three years of acute and community nursing experience
  • Demonstrated interest in community nursing and primary healthcare
  • Clinical experience in:
    • Wound management
    • Central Venous Access Devices (CVADs)
    • Palliative care
    • Negative Pressure Wound Therapy
    • Stoma care
    • Catheter management

  • Ability to work independently with minimal supervision
  • Strong decision-making and problem-solving skills
  • Experience collaborating within a multidisciplinary healthcare team
